We are seeking a motivated and compassionate Discharges Core Trainee/ST1&2 (formerly SHO) to join our dynamic healthcare team. As a Discharges Core Trainee, you will play an essential role in coordinating patient discharges, ensuring that all necessary assessments, documentation, and follow-up care plans are effectively managed. The ideal candidate will have completed their Foundation Year 1 and be in the process of completing their Foundation Year 2 training, possessing strong clinical knowledge and excellent communication skills. Key responsibilities include conducting thorough patient assessments, developing discharge plans, and educating patients and families about ongoing care needs. You will also be expected to participate in ward rounds, attend discharge planning meetings, and support the implementation of quality improvement initiatives related to discharge processes.
