In this position, you will collaborate closely with multidisciplinary teams to assess, diagnose, and implement appropriate treatments for patients experiencing frailty. Your day-to-day responsibilities will include conducting thorough clinical assessments, developing individualized care plans, and actively engaging in patient safety initiatives. To be successful in this role, you should possess a medical degree and have completed a foundation training program or equivalent. Experience in acute medical settings, particularly with elderly patients, will be advantageous but is not a strict requirement.
